• Employee makes request 30 days prior, or as soon as leave is known
    • Complete Staff leave request (SLR)
  • Employee submits signed SLR contacts HR representative
  • Department will determine Eligibility for FMLA Leave
  • Meeting will be set up between employee and HR representative to go over leave dates
  • HR representative will give additional documents to employee for completion
  • Upon return, employee must have release documents from doctor



  • Employee must have at least 12 months of University service (need not be continuous) and at least 1,250 hours actually worked in the 12 month per iod immediately preceding commencement of the leave (hours actually worked includes overtime, but does not include holiday, vacation, sick leave, or other paid leaves).
